Health Assessment

Welcome to THRIVE!

An evidence-based wellness course shown to successfully support students in growing, experiencing, and sustaining their well-being and life satisfaction.

Before we begin the course, you’re invited to complete this optional but recommended comprehensive survey. Once completed, this platform will give you a personalized report of your current health metrics and summary of your top strengths and recommended areas for growth that this course can help you build. The survey is estimated to take about 10-15 minutes to complete and is highly recommended to help you see your specific areas of strength and areas of opportunity to thrive. If you complete this survey, at the end of the THRIVE program, you will be invited to complete this survey again, and you will be shown how your health metrics changed over time, possibly as a result of changes you made while taking this course. Completing this survey is optional. You may decline to complete this survey and still participate in the THRIVE program, however if you decline to take this survey, you will not be provided with any personalized recommendations tailored to you during this program.

 

All personal health information, survey data, and results you provide will be private and protected according to HIPAA and THRIVE's Privacy Policy and Terms & Conditions. Your school and instructor will only be shown grouped, deidentified survey results for your entire class (for example, the average overall anxiety score for your entire class) as indicators of overall health and progress. Your school and instructor will not see any of your individual results, and your individual responses will never be shared with any third parties unless you provide an indication in your survey results that you may be in crisis or a danger to yourself or others, in which case your instructor will be notified in order to provide additional support.
