What information do we collect and use, and why?
Waiver and Consent Agreement Form
Our online Waiver and Consent Agreement Form (https://www.discoverhowtothrive.com/consent) is provided to alert individuals to the terms and conditions required to participate in the THRIVE Program and to verify the understanding and agreement of those who elect to participate in the Program to assume all risk and responsibility for their participation. We collect the participant’s name, date of birth, email address, phone number, program provider, and program start date. If the Participant is a minor under the age of 18 years old, we also collect the minor’s guardian’s name, email address, and phone number in addition to the information previously listed. We collect contact information to be able to contact potential and existing program participants if needed to verify their eligibility to participate, answer questions, or address any arising needs or concerns. Unless you ask us not to, we may also use this information to contact you via email in the future to tell you about specials, new products or services, or changes to this Privacy Policy. We collect date of birth to verify eligibility to consent and participate. We collect Program provider and Program start date to share with the third-party provider the list of individuals enrolled in their Program offering who agreed to these terms and conditions and who are therefore authorized to participate in the Program. For this purpose, we provide the provider with all of the information the individual submitted on this form. The information collected is stored in a secure, password-protected Wix database. You may not request to have this information deleted from our database after it is submitted to us because they are indicative of a legal binding agreement between you and THRIVE Institute LLC confirming your consent to participate in a THRIVE Program.
Feedback Form
Our online Feedback Form (https://www.discoverhowtothrive.com/feedback) allows us to collect information from THRIVE Program Participants about their experience and any input, suggestions, or reactions they have regarding their participation. This information is collected to enhance our products and services, to improve customer support, for marketing and promotional purposes, and to report results to third parties. Only anonymous, deidentified information submitted in this form will be shared with third parties. Users have the option to provide their first name and/or last name in this Feedback Form, which makes their data identifiable. Users are not required to provide their name. If you do not wish to have your feedback linked to your name, do not complete the name fields. We give users the option to provide their name to allow us to contact the participant for reasons including but not limited to answering questions or responding to an unsatisfactory experience. Information submitted into this Feedback Form is stored in a secure, password-protected Wix database. You may request for this information to be deleted from our database at any time.
Online Store

This information is collected for billing purposes, to fulfill your order, to contact you regarding any questions or issues, and to verify your agreement to the terms and conditions binding you to the License Agreement required to use our products. Transactions are managed by Stripe, Inc., a software platform that handles billions of dollars in financial transactions every year across the world.

How do we protect your information?
We take precautions to protect your information. When you submit sensitive information via the website, your information is protected both online and offline.
Wherever we collect sensitive information (such as credit card data), that information is encrypted and transmitted to us in a secure way. You can verify this by looking for a lock icon in the address bar and looking for "https" at the beginning of the address of the Web page.
Data and information you provide on our site may be stored through Wix’s data storage, databases, and the general Wix’s applications. Wix stores your data on secure servers behind a firewall.
All direct payment gateways offered by Wix and used by our company adhere to the standards set by PCI-DSS as managed by PCI Security Standards Council, which is a joint effort of brands like Visa, MasterCard, American Express, and Discover. PCI-DSS requirements help ensure the secure handling of credit card information by our store and its service providers.
Wix does not retain, share, store or use personally identifiable information for any secondary purposes beyond those shared in this policy.
While we use encryption to protect sensitive information transmitted online, we also protect your information offline. Only employees who need the information to perform a specific job (for example, billing or customer service) are granted access to personally identifiable information. The computers/servers in which we store personally identifiable information are kept in a secure environment.
We will not sell or rent your personal, identifiable information with or to any third party outside of our organization, other than for purposes including but not limited to fulfilling your request, security or ensuring the safety or well-being of any person.

How do we share information with third parties?
We partner with third parties to provide specific services, such as our partnerships with institutions and organizations who offer the THRIVE Program. We may provide these third parties with the identifying information they need to provide these services. For example, an institution may request from the Company a list of the names of participants who completed our online Waiver and Consent Agreement Form to verify the individuals who are permitted to participate in the THRIVE Program offered. These third parties are contractually bound to protect any shared personally identifiable information and are not allowed to use the shared personally identifiable information for any purposes other than providing these services.
For purposes including but not limited to reporting and promoting our company results and progress, we also share information and data collected on our website and within our email account with third parties such as our existing clients, potential clients, and potential advertisers or investors. We will only share with third parties anonymous data and deidentified information that cannot be linked to identify any individual person. We report data in such a manner that no one can identify individual participants or recognize their information. For example, we will share deidentified, anonymous comments and aggregated participant demographics that THRIVE Program participants submit in our online Feedback Form and Waiver and Consent Agreement Form.
