To download and use the THRIVE curriculum materials to run the THRIVE Program at your institution or organization, you must purchase a license.
Includes:

- the online THRIVE Program Consent Form
- the online THRIVE Program Feedback Form
The curriculum license is valued at $5,000.
The total purchase price includes credit card processing fees ($159). Contact us for alterative payment method options that do not include this fee (such as a mailed check) .
A 6% sales tax ($300) has also been added to the purchase price of the license. If you have tax-exempt status, please email us at info@discoverhowtothrive.com for a coupon code that will remove the additional sales tax fee from your purchase price. The coupon code will be provided to you once we receive from you via email a tax exemption certificate acceptable to the applicable tax authorities evidencing your exempt status.
If you are located outside of Pennsylvania, please email us at info@discoverhowtothrive.com for a coupon code that will remove the sales tax fee from your purchase price. Please note that you, the buyer, shall be responsible for any and all applicable taxes or fees associated with or pertaining to this purchase, such as the payment of use tax.

Upon purchasing a license to use this product, you agree that neither you nor any member of your organization will copy, edit, share, email, or distribute in any way this product in part or as a whole. Violation of the license shall subject your organization to all remedies available at law and equity.
